Before Your First Visit
Please complete your intake forms through the patient portal before your first appointment when possible. On the day of your visit, eat a light meal within a few hours beforehand, stay adequately hydrated, and wear comfortable clothing that can be adjusted as needed for treatment.
For your first appointment, plan to arrive a few minutes early so you have time to locate Suite 125 and be ready when your appointment begins.
